日本（にほん）では
どの病院（びょういん）にも自由（じゆう）に行（い）くことができます。
→ これを「フリーアクセス」といいます。
今（いま）でも、かかりつけ医（い）を決（き）めなくてもかまいません。

You do not have to choose a family doctor if you do not want to.

新（あたら）しい制度（せいど）はいつから？
2025年（ねん）4月（がつ）から、「かかりつけ医（い）」の制度（せいど）が少（すこ）しずつ新（あたら）しくなりました。
名前（なまえ）は「かかりつけ医（い）機能（きのう）報告制度（ほうこくせいど）」です。

What is new?
① かかりつけ医（い）を決（き）める紙（かみ）があります
「この先生（せんせい）にいつも相談（そうだん）します」と書（か）いた紙（かみ）にサインします。
紙（かみ）は病院（びょういん）で保管（ほかん）されます。
① You can sign a paper to choose your family doctor
You write, “I will see this doctor regularly.”
You sign the paper.
The hospital or clinic keeps the paper.
② どの病院（びょういん）がかかりつけ医（い）をしているか、インターネットで調（しら）べられます
かかりつけ医（い）として対応（たいおう）している病院（びょういん）を、ネットで探（さが）せるようになります。
② You can check online which clinics offer family doctor services
You can search online to find out which clinics and hospitals have family doctors.
③ 大（おお）きな病院（びょういん）に行（い）く前（まえ）に、まずかかりつけ医（い）へ行（い）くことが多（おお）くなります
必要（ひつよう）なときは、紹介状（しょうかいじょう）を書（か）いてくれます。
紹介状（しょうかいじょう）がないと、大（おお）きな病院（びょういん）で追加（ついか）の費用（ひよう）がかかることもあります。

If needed, your family doctor writes a referral letter.
If you go to a big hospital without a letter, you may have to pay extra money

同（おな）じ先生（せんせい）に、からだのことをよく知（し）ってもらえます。
大（おお）きな病院（びょういん）が混（こ）みにくくなります。
病気（びょうき）のとき、まず相談（そうだん）する先生（せんせい）がいると安心（あんしん）です。
Why is this system good?
One doctor can get to know your body and health well.
Big hospitals won’t be so crowded.
It is easy to get help when you are sick.
